OB claims continue to be prevalent across the nation but more importantly carry the highest severity. Although all adverse outcomes cannot be prevented, the moral and economic ramifications involving OB claims are so great that understanding the basic tenets of a high reliability unit (HRU) is paramount to affect positive change in this area.
High reliability includes creating a culture and processes that radically reduce system failures and effectively respond when failures do occur. Creating, but more importantly sustaining, an obstetrics HRU takes diligence, standardization, and leadership support.
