Although all adverse outcomes cannot be prevented, the moral and economic ramifications involving OB claims are so great that understanding the basic tenets of a high reliability unit (HRU) is paramount to affect positive change in this area.

High reliability includes creating a culture and processes that radically reduce system failures and effectively respond when failures do occur. Creating, but more importantly sustaining, an obstetrics HRU takes diligence, standardization, and leadership support.
