Reducing unnecessary or preventable hospital readmissions is a quality indicator affecting payment in every health care venue. Skilled nursing facility (SNF) providers play a key role in decreasing readmission rates and both during the patient’s stay as well as after discharge from the SNF. Central to reducing readmissions is a coordinated, interdisciplinary effort focused on disease management, fall prevention and management, and ensuring coordinated transitions between the referring hospital/physician and the facility as well as between the SNF and the patient’s next level of care after discharge. This presentation outlines several tangible actions skilled nursing facility providers can take to positively impact their overall readmission rate and improve the outcomes of the patients they serve.
